United & Empowered Care, Inc. operates five licensed Provider Residential Group Homes in the City of Hampton, VA. Each home is modern, welcoming, and staffed by certified professionals dedicated to creating a safe and nurturing environment.
Our Residential Group Homes are designed to support individuals in building and maintaining meaningful relationships with peers, family, friends, and staff. We carefully match residents to create compatible living arrangements that foster positive peer-to-peer emotional support. United & Empowered Care proudly serves individuals across the full spectrum of needs , from high-functioning to those requiring more intensive support.
We provide care in a family-like setting, offering 24-hour staffing, seven days a week, for male and female adults with intellectual and other developmental disabilities with high-functioning abilities and intellectual developmental disabilities. In addition to daily support, we offer Skilled Nursing Services to meet a variety of medical needs.
Our focus is on helping each individual maintain independence and dignity as their support needs evolve. We are committed to ensuring that every resident has the opportunity to live their best life — with the care, respect, and support they deserve.
Day Support Program
The Day Support Program was established to provide individuals with intellectual and developmental disabilities a supportive environment in which to pursue their goals and reach their full potential. Our dedicated staff offers social skill building, peer interaction, life skills development, coping strategies, and health and safety education — all tailored through individualized training plans.
Participants in the program engage in group activities designed to build awareness and understanding of current events. They also learn essential independent living skills, such as cooking, personal hygiene, and setting healthy boundaries, which foster greater social interaction and personal growth.
The United & Empowered Care Day Support Program includes sensory rooms for individuals who prefer a calm, low-stimulation environment. In addition, computer access is available to support individuals as they learn to use technology safely and effectively.
United & Empowered Care, Inc. also offers Skilled Nursing Services to meet the medical needs of participants. These services include medication management, G-tube feeding, nutrition management, diabetic care, and vital sign monitoring, all provided by licensed nurses and trained staff.
Community Engagement & Community Coaching
The United & Empowered Care, Inc. Community Engagement & Community Coaching Program empowers individuals to participate in a variety of community activities, including actively. This program is designed to promote independent, goal-oriented growth in a safe and supportive environment.
Participants have numerous opportunities to develop independent living skills while engaging with community members such as police officers, firefighters, postal workers, hospital personnel, vocational employees, and other local professionals. Through these interactions, individuals learn essential community safety rules, practice appropriate social interactions, and build a foundation for educational and financial development.
The program also encourages participants to give back to their community through volunteer opportunities with organizations such as Meals on Wheels, the Red Cross, and voter registration initiatives.
Ultimately, the Community Engagement & Community Coaching Program focuses on skill development for potential workforce participation, fostering greater independence, confidence, and social connections that prepare individuals for meaningful roles in their communities.
Sponsored Residential
The United & Empowered Care, Inc. Sponsored Residential Program provides compassionate care and support to adults and adolescents with intellectual and developmental disabilities in both home and community settings. This program creates a comfortable, safe, and empowering living environment where individuals are encouraged to make their own choices and develop essential life skills.
Sponsored Residential Providers offer guidance in areas such as budgeting, personal hygiene routines, household cleanliness, fire safety, and cooking. The program is designed to focus on the individual’s unique needs and goals, promoting growth, independence, and overall quality of life. It supports individualized outcomes by addressing key areas, including choice, relationships, lifestyle, health, well-being, human rights, and life satisfaction.
United & Empowered Care, Inc. delivers 24-hour Sponsored Residential Services for individuals diagnosed with intellectual and developmental disabilities (ID/DD). Providers work closely with individuals to help them develop the critical life skills necessary to live as independently as possible within their communities.
Each participant follows an Individualized Service Plan (ISP), tailored to a person-centered approach, ensuring that support is aligned with their personal goals, preferences, and strengths.
